摘要

台灣癌症醫學發展與制度的建立，需要有新觀念與技能的落實，尤其是醫療的發展與社會信賴關係的營造密不可分，這也是所有醫療從業人員所需遵守醫療社會契約的本質。本文爲癌症醫學所所贊助台灣癌症治療口述歷史計畫的一部分，透過於全國各地癌症治療專科醫師的訪談，回顧癌症治療臨床試驗發展制度建立的過程所蘊含的醫病關係。本文首先勾勒台灣腫瘤醫學臨床試驗發展的緣起，而後呈現1989年「台灣癌症臨床研究合作組織」（Taiwan Cooperative Oncology Group，簡稱TCOG）成立與1990年代癌症醫學會建立腫瘤內科專科醫師制度之後，我國醫學中心與世界癌症臨床試驗發展的趨勢，最後藉由腫瘤專科醫師於醫療制度與醫病關係的反思，提出共同營造信賴關係，開展國際機會的前景。

Contextualizing new ideas and technologies has been crucial foundation for the development and institutionalization of medical oncology in Taiwan. This development is closely related to social trust building and has become the essence of medical social contracts. This paper is part of the 'Taiwan Oncology Oral History Project,' sponsored by Chinese Oncology Society in Taiwan. The doctor-patient relationship embedded in the institutionalization of cancer clinical trials are explored. We first delineate their origins. Thereafter, the emergent trends of leading medical centers in Taiwan and cancer clinical trials at global scale are presented. Finally, perspectives on trust building and international opportunities are proposed with oncologists' critical reflections on the health care system and doctor-patient relations.
