摘要

本研究以急診就醫的跌倒老人為研究對象，嘗試瞭解健康相關生活品質中生理、心理、社會、環境等範疇以及跌倒後心理害怕之影響因子。研究對象為2005年1月至2005年6月在台北市立聯合醫院忠孝院區及中興院區、新竹馬偕醫院、中國醫藥大學附設醫院等因跌倒就醫且年齡在六十五歲以上的老人。半年研究觀測期間之急診室登錄有874位跌倒老人，其中410位於急診後一個月同意接受電訪完成世界衛生組織生活品質問卷與特定活動平衡信心量表等問卷。多變項線性迴歸分析結果顯示慢性病況數以及基本與工具性日常生活能力會影響生理、心理、及社會三個範疇之健康相關生活品質；而跌倒種類、輔具使用、認知功能、基本及工具性日常生活能力則會影嚮環境範疇。另外，基本日常生活能力會影響跌倒後的害怕心理。本研究結果有助於了解老人跌倒後各個面向的健康相關生活品質及心理之害怕跌倒程度，可做為對老人跌倒介入內容與方向之參考。

This study aimed at examining the factors associated with the four domains of the WHOQOL-BREF (a brief version of the World Health Organization Quality of Life) as well as the fear of falling in elderly people aged 65 and over. During a 6-month recruitment period, there were 874 elderly fallers visiting emergency rooms in the three hospitals: Taipei City Hospital (Zhongxiao Branch and Zhongxin Branch), Mackay Memorial Hospital (Hsinchu Branch), and China Medical University Hospital. Of these people, 410 were interviewed by telephone to collect essential information. Results of the linear regression model analysis show that three WHOQOL-B REF domains-Physical Capacity, Psychological Well-being, and Social Relationships, were associated with the number of chronic conditions and limitation of basic and instrumental activities of daily living (ADLs). On the other hand, environmental domain was associated with cognitive impairment, fall patterns, use of walking device, and limitation of basic and instrumental ADLs. Furthermore, fear of falling was associated with limitation of basic ADLs. The results are conducive to the understanding of psychological state and the design in the contents and direction of intervention for elderly fallers.
