A Look Forward of Integrative Approaches for Oncology Patients Receiving Chemotherapy at a traditional Chinese Medicine Ward in Taiwan

腫瘤患者接受化學治療在台灣中醫病房的整合醫療之展望

摘要

Objectives: To describe the integrative treatment approach for cancer patients receiving chemotherapy provided at Kaohsiung Chang Gung Memorial Hospital. Summary: The traditional Chinese medicine ward at Kaohsiung Chang Gung Memorial Hospital has been treating cancer patients with an integrative approach for the past 10 years. Within hours or days of anticipatory symptom onset before chemotherapy, benzodiazepines or 5-Hydroxytryptamine type 3 receptor antagonists are recommended, but traditional Chinese medicine ward treatments such as auricular acupuncture, massage, and music therapy can be used to help tranquilize the nerves and provide homeostasis of the stomach-qi. In the initial and early treatment stages, several types of acupuncture are used depending on individual conditions. Cupping and moxibustion help to restore gastrointestinal dysfunction, alleviate pain, and clean inflammatory processes. A Qigong program is also applied to help relieve mental stress and limb paralysis. If patients suffer from delayed symptoms of chemotherapy, laser or abdominal acupuncture and Chinese herbs are utilized. Conclusion: Despite the need for rigorous research, treatment options are limited, so we strongly recommend the use of integrative medical care to alleviate the side effects of chemotherapy.

目地：本文為化療病患在台灣中醫病房接受整合醫療服務模式之經驗提供。摘要：如何提供癌症患者最完善的照顧一直是個重要的議題。化學治療時常伴隨許多副作用，而現代醫療結合傳統中醫療法之整合醫療模式已蔚然趨勢。本院設立中醫病房推動中西醫整合醫療，來緩解抗癌治療過程中的不適，並累積照護經驗至今超過十年。化療前若發生預期性嘔吐，除了給予苯二氮平類或5-羥色胺3受體拮抗劑外，可考慮耳針、推拿、五音療法等來穩定情緒及平降胃氣。在化療初期階段或急性期，不同形式的針灸、以及推拿、或拔罐可用來緩解腸胃道副作用、減輕疼痛、和降低發炎反應。氣功亦可用於心靈上或肌肉麻木等的問題。延遲性化療副作用則以中藥治療、雷射針灸、或腹針為主。結論：我們可藉由中西醫的整合醫療模式，來減少癌症治療所伴隨的副作用。然而，未來仍需更具規模的研究來證實，以提供更大的助益。
