Squamous Odontogenic Tumor – A Review of 41 Reported Cases
鱗狀齒源性腫瘤— 41例文獻回顧
摘要
鱗狀齒源性腫瘤是一種良性上皮性齒源性腫瘤，顎骨中鱗狀齒源性腫瘤可能源自牙周膜中馬拉塞氏上皮殘留體，而牙齦中鱗狀齒源性腫瘤可能源自牙齦表面上皮或牙板殘留上皮。組織學上，此腫瘤包含多個分化良好鱗狀上皮小島分散於纖維基質中。至目前為止，於英文文獻中共有41個可接受之鱗狀齒源性腫瘤病例。此腫瘤好發於21至30歲患者，男性之發生率略高，男女患者比為1.7:1。鱗狀齒源性腫瘤於上、下顎之發生率約略相同，於上顎約平均分佈前牙區和後牙區，於下顎好犯後牙區。大部分之鱗狀齒源性腫瘤發生於顎骨中，文獻中只有二例發生於牙齦。顎骨中之鱗狀齒源性腫瘤常呈現包圍牙根或於牙根側面半圓形成三角形之放射線透射區域。鱗狀齒源性腫瘤通常以剜出術，刮除或局部切除治療，但位於上顎之鱗狀齒源性腫瘤，因其較具侵犯性，常須以更徹底之手術如部分上顎切除術治療。鱗狀齒源性腫瘤之預後良好，至目前為止只有一例再發病例。然而文獻中也有一例下顎鱗狀齒源性腫瘤惡性轉變為鱗狀細胞癌之報告病例。

Squamous odontogenic tumors (SOTs) are benign epithelial odontogenic tumors probably arising from the rests of Malassez in the periodontal ligament for central type lesions and from the gingival surface epithelium or remnants of dental lamina for peripheral type lesions. Histologically, SOTs are composed of islands of well-differentiated squamous epithelium in the fibrous stromata. To date, 41 confirmed cases have been reported in the English literature. SOTs occurred more frequently in the third decade of life. There was a slight male predilection for SOTs, with a male-to-female ratio of 1.7 to 1. SOT lesions were distributed approximately equally between the mandible and maxilla. In the maxilla, lesions were evenly distributed in the anterior and posterior segments, whereas in the mandible, the premolar-molar region was affected more often. The majority of reported SOTs occurred centrally in the jawbone. Only 2 cases were located peripherally on the gingiva. Radiographically, central type SOTs appear as a semicircular or roughly triangular radiolucent lesion around or lateral to the roots of teeth. SOTs were frequently treated by enucleation, curettage, or local excision. Tumors located in the maxilla might need more-radical treatment including a partial maxillectomy because of a more-aggressive biological behavior. The prognosis with this lesion is good. Out of 41 cases, only 1 recurrence was reported; however, malignant transformation into a squamous cell carcinoma from a case of mandibular SOT has also been documented in the literature.
