The mandibular first premolar, with its wide variation in root canal morphology, is generally agreed to be the most difficult tooth to treat endodontically. The presence of a C-shaped canal system may be a challenge for carrying out the traditional root canal treatment. The expression of a C-shaped canal system may be more common in peoples of Asia. The prevalence of a C-shaped canal mandibular first premolar ranges from 10%-17% according to Sikri et al and Lu et al. This article represents 2 case reports regarding the C-shaped canal morphology at the apical third of 2 manbdibular first premolars. The clinical symptoms and signs of the patients improved after they underwent apical root resection with retrograde cleaning and shaping, super EBA filling and intentional replantation.
