全民健保實施前軍眷之認知、態度及參與意願

Perception and Attitudes of Military Dependents toward the National Health Insurance

摘要


由於軍眷在全民健保實施之前無須繳納保險費,赴各級軍醫院就醫時也享有完全免費之醫療服務;但在實施全民健保之後,依法不僅須繳納保險費,就醫時除必須受轉診制度之約束,也須部份負擔醫療費用。為瞭解軍眷於面臨經濟負擔增加以及就醫習慣重大改變時,對即將實施的全民健保之認知情形、態度及參與意願,本研究乃於全民健保實施前,以系統隨機抽樣方法抽取8,148位軍眷家戶代表,採郵寄問卷法蒐集所需資料;共回收3,145份問卷,實際分析有效問卷數為3,089份。綜合本研究之結果發現:軍眷受試者對於全民健保的諸多規定仍缺乏應有之基本知識,其認知平均得分僅為6.53分(總分為14分);在一般態度方面,大部份的受試者並不同意於繳交保險費之外,須再負擔額外的部份醫療費用及受轉診制度的限制;而且絕大部份的軍眷希望沿襲以往的就醫習慣與待遇,即赴各級軍醫院就醫仍維持完全免費而且不必轉診;在整體技持態度方面,則有半數以上(56.8%)贊成實施全民健保;此外,仍有42.7%的受試者表示願意加入全民健保。由此可見,本研究中的軍眷受試者對於全民健保的認知情形雖然呈現者普遍茫然的現象,但是對於全民健保的實施,仍寄予相當高的支持與期望。

Before the implementing of the National Health Insurance (NHI), military dependents in Taiwan have the benefit of free access to medical care provided by the military medical care systems. After the NHI being put into practice, the military dependents begin to face a 40% coinsurance rate and a required copayment when seeking for medical services. The purposes of this study were to scrutinize impacts of the NHI on military dependents in terms of their perception of, attitudes toward, and willing-to-enroll the NHI. Study samples were drawn from the Ministry of Defense military dependents database. Through systematic sampling, we chose 8,148 military family representatives as our study subjects. Questionnaires were mailed to subjects and 3,145 returned with a response rate of 38.6%. After removing incomplete and wrong-person's questionnaires, a total of 3,089 cases were included in the analyses. Military dependents were not perceiving of all NHI regulations concerned them. In average, they learned about half of the regulation items. Less than 30% of military dependents agreed on copayment and referral system regulated by NHI. However, 56.8% of the dependents showed their support to NHI as a whole, and 42.7% even chose to join the NHI under the assumption that they had the option. The study concludes that most military dependents are with poor perception of the NHI but hold great expectations and positive attitudes toward the implementing of NHI Plan.
