Migration of Gastric Bezoar with Recurrent Bowel Obstruction: Report of a Case
胃糞石的移動以及復發性糞石阻塞：病例報告
摘要
糞石造成腸阻塞是很少見的機械性腸阻塞，而復發性者更是少見。我們報告一個病例是遺留胃糞石所造成的術後立即復發腸阻塞。因爲在術前的評估時，症狀不明顯，所以在第一次腹部電腦斷層影像上沒有注意到這個胃糞石，因此第一次手術僅針對造成腸阻塞的腸糞石。 之後因爲開完糞石碎裂術後七天又發生腸阻塞，由第二次的電腦斷層影像推測，是由於胃糞石移動到腸道所引起的。回顧文獻紀錄，術中進行胃糞石碎裂術是可以避免再次腸阻塞的做法。

Bezoar-related bowel obstruction is one unusual cause of mechanical ileus, and recurrent bowel obstruction with a bezoar is even rarer. We present a case of recurrent postoperative bezoar bowel obstruction due to a missed concomitant gastric bezoar, which was shown in the pre-operative abdominal CT, but in this case the main lesion related to bowel obstruction had been considered to be the bowel bezoar. A second operation was performed 7 days later for the recurrent bowel obstruction. A review of the literature shows that intra-operative fragmentation of the gastric bezoar should be considered to prevent recurrent bowel obstruction.
