A rare case of an irreducible lateral dislocation of the elbow with concomitant fractures of the lateral humeral epicondyle and capitellum is presented. After failed attempts at closed reduction, open reduction was performed immediately. The reduction was still not achieved easily until the obstructing band of brachialis muscle was counteracted by means of an elevator instrument. It ended up arthrosis and joint contracture, ascribed to severe soft-tissue injury and intraarticular fracture.
