Cervical Pin Migration Following Open Reduction and Pinning of a Distal Clavicular Fracture: A Case Report

Fractures of the clavicle are common bony injuries and account for 10%-12% of all fractures. Among the three groups of clavicular fractures, distal clavicular fracture may have a high incidence of nonunion, and surgery is required if there is an open fracture, gross displacement, neurovascular injury or floating shoulder. We report on a case in which the pin migrated through the mid-cervical posterior column, following open reduction and internal fixation of a fracture of the distal clavicle. Pin migration following surgery on the shoulder girdle is more common than in other locations. Reports of wires and pins migrating to other distant sites have been published. No adequate explanation has been given but, in our case, shoulder overuse was detected as a result of crop cultivation by the patient who was a farmer. When deciding to perform an open reduction and internal fixation, the better choice is to use a plate. A threaded pin is a preferable choice if one needs to use pinning. In addition, avoid moving the pin to-and-fro and do not forget to bend the pin laterally.
