Stabilization means placing a diseased or painful spinal segment in a pain-free, balanced or neutral position and holding it there while performing any task. It is a retraining program of all muscles which control the motion and position of the lumbar spine and pelvis. The exercises emphasize the concept of muscle fusion or abdominal bracing which involves the cocontraction of the abdominal muscles and trunk extensors to maintain a corsetting effect on the spine. Through the basic and advanced training courses, the patient is taught to find his neutral position and performs each exercise in this position. The exercise must be done precisely and repeatedly so that the brain can engram the proper spinal movement pattern. According to the literature, many authors presented their rationale of stabilization exercises from different points of view. This paper is aimed to introduce and summarize the concept and the scientific rationale of stabilization. A specific program including both basic and advanced levels is also presented.
