低能量雷射對壓瘡治療之效果：病例報告

本研究的目的在於探討使用低能量雷射(low energy laser)促進壓瘡(Pressure sore)傷口癒合之效果。針對三位患有骶骨(sacrum)部位壓瘡的病人，除予傷口清潔換藥之外，並使用錄鋁鉀半導體(Ga-Al-As diodes)曾射儀器每週照射三次雷射光。對於評估傷口的癒合情形，遊治療前與治療徒每週均使用照相機拍攝，以記錄傷口之大小，再利用Auto-Cad軟體計算傷口的表面積，做為治療效果的比較。第一例經六周之治療，傷口有87.8%癒合；第二例經十周治療，有99.4%癒合；第三例在雷射治療期間，曾接受擴創手衍(debriement)，且在十一周後出院，雖僅有23.4%之傷口癒合，但從外觀上可見傷口向深變淺，肉芽組織增生(granulation)情形良好，顯示該傷口正在癒合中。尚上迷三例可知，在臨床上對於壓瘡的處理，低能量雷射提供了新的治療方向。

The purpose of this study was to investigate whether the low energy laser therapy promote the healing of pressure sore. Three patients who had pressure sores over the sacrum areas were treated with Ga-Al-As semiconductor diodes laser (Biotherapy # 2001, Omega Universal Technologics, Ltd) and dress-changing three times per week for a length of 6-11 weeks. To evaluate the effects on wound healing, slides were taken over the areas with pressure sore once a week for each patient. Then, the slides were projected onto papers, on which the margins of wounds were traced. Finally, the areas within the margin of the wounds were calculated with a digitizer and the Auto-Cad software. Patient one received the laser therapy for a length of 6 weeks. There was 87.8% decrease in area of pressure sore when compared with the original size of the wound. Patient two demonstrated 99.4% healing of the pressure sore with the laser therapy for 10 weeks. Patient three showed only 23.4% healing of the pressure sore after 11-week laser therapy. However, notable granulation tissues were found within the wound in patient three. Based on the above findings, the healing of the pressure sore seemed to be promoted by the low energy laser therapy. This study suggests that the low energy laser therapy is a potential modality in treating patients with pressure sore. Future studies with control groups are recommended to explore the effects of the low energy laser therapy on the wound healing of the pressure sore.
