The Study of Local Circulation and Temperature of Osteoarthritic Knees

膝部骨性關節炎局部血液循環及表皮溫度之研究

摘要


本實驗的目的有三:(一)希望能明白膝部骨性關節炎患者其膝關節附近的血液循環是否與同年齡層的健康膝有所不同。(二)又與年輕者的健康膝有何差異?(三)希望了解這些膝部骨性關節炎患者,其膝關節的表皮溫度是否較正常膝之溫度爲高? 自民國81年2月至民國82年4月,作者總共從臺大醫院健科與骨科門診選擇了63位膝部罹患骨性關節炎的患者爲研究的對象。另外選擇2組健康膝爲控制組,一爲年輕者,共63位;一爲年齡與骨性關節炎患者相仿的中、老年人,共52位。三組受試者皆接受股動脉、膕動脉的血液循環測試,也接受膝關節表皮溫度的測試。利用朴卜勒超音波測試儀(Doppler ultrasound, Diasonics DRF 400)測試股動脉及腘動脉的血管直徑,血管橫切面面積(area of cross section),單位時間面積的血流速(time average velocity),和每分鐘的血流量。利用溫度測試儀(TS 120 Thermosentry)在膝蓋中心點,膝上15公分、膝下10公分處擷取表皮之溫度。實驗組的患者如果兩膝皆罹患骨性關節炎,只測量症狀較嚴重的患膝。其餘兩組控制組皆以右膝接受測驗。 實驗結果如下:1.若以血管直徑、血管橫切面面積言之,三組之間並無明顯差異。2.若以單位時間面積血流速及每分鐘之血流量言之,則年輕受試者之量遠大於年長者,包括健康膝或骨性關節炎患膝,但是兩組年長者彼此却無差異。3.年長者(包括健康膝及骨性關節炎患膝)其膝蓋中心點之表皮溫度相較於年輕者爲高,同樣的,兩年長組之間的差異幷不明顯。膝上15公分與與膝下10公分處的表皮溫度,三組之間皆無明顯差別。

The purposes of this study were 1) to realize whether the local circulation was different between subjects with osteoarthritic and healthy knee in matched age group; 2) to know whether the osteoarthritic patients differed from the young subjects in circulation of knee joint: 3) to understand whether the temperature around the knee joint was higher in osteoarthritic patients. The vessel diameter (cm), its area of cross section (cm^2), the time average velocity (cc/cm^2/sec) and total volume of blood flow (cc/min) of femoral as well as popliteal arteries were assessed by Doppler ultrasound. The local skin temperature of knee joint was evaluated by a thermometer. After data analysis, the study demonstrated the following The results were: 1. The vessel diameter of femoral or popliteral artery was not significant ly different among the three groups, and the area of cross section was not different, either. 2. The blood flow, both average time velocity and total volume per minute was less in elderly subjects including healthy and osteoarthritic groups. The difference of blood flow was significant between young and aged groups. 3. The difference of blood flow was not significant as the healthy group in comparison with the osteoarthritic groups. 4. There was significant difference between young and aged population in the skin temperature at central patellar point. But there was no significant difference between older healthy and osteoarthritic groups.
