穴位埋線對單純性肥胖女性身體質量指數的影響:一個隨機、單盲試驗

The Effect of Acupoint Catgut Embedding Treatment on the Body Mass Index of Simple Obese Females: A Randomized, Single-Blind Trial

摘要


隨著國人健康意識的抬頭,許多肥胖患者開始接受減肥治療,其中穴位埋線已成為潮流,但大部分的論文文獻缺乏實驗設計之嚴謹性。本研究納入20~40歲女性且BMI介於24~33kg/m^2之單純性肥胖患者57人,依照隨機分派且單盲之方式,分為穴位埋線組及穴位空針組(僅空針穿刺,無埋入羊腸線),每週接受一次治療,共計5次治療,比較5週後兩組BMI之差異。結果:131位進入篩選後,僅57位參加臨床試驗,其中擅自使用其他減肥方式或其他原因致使退出研究有13人,完成療程及追訪之受試者共44人,包括穴位埋線組24人,穴位空針組20人。觀察治療前後BMI的變化,發現穴位埋線組BMI下降且達統計學意義(p<0.05),隨治療次數的遞增,BMI下降越多,表示穴位埋線治療單純性肥胖有一定的療效。穴位空針組雖經過空針刺激,對穴位僅短時間刺激作用,即使BMI有下降,但5周內無法達到統計學意義(p>0.05)。穴位空針組和穴位埋線組之組間比較無統計學差異,表示空針與埋線穿刺穴位5周內效果接近。結論:本研究受限於嚴謹之收案標準、患者拒絕接受隨機分派及高退出率,使得最後之樣本數目太小,穴位埋線治療單純性肥胖女性之療效仍不確定,但初步資料顯示,可能存有潛在的效益。

Public awareness of the impact of obesity on health is rising. Acupoint catgut embedding treatment for obesity has become popular, however, seldom of those papers had strict experimental design. This study was a clinical randomized, single-blind, controlled trial. A population of 57 female with simple obesity has BMI between 24-33kg/m^2 and aged between 20 and 40 years enrolled in this study. They were arranged randomizedly into the study and control group. The control group received acupuncture without catgut embedding treatment, however the study group received both. Both groups received intervention once per week for 5 weeks. The BMI was compared with each other after 5 weeks. A total of 131 people entered the screening, and then 57 female with simple obesity met the eligibility criteria were enrolled. 13 of them withdrew because of using other weight loss methods, etc. Finally, 44 subjects completed the study, including 24 from the study group and 20 from the control one. The BMI of study group subject showed reduction week by week, but became statistically significant at the 5th week. The control group received shorter duration acupoint stimulation without prolong stimulation by catgut embedding treatment, however, the BMI had still descended but just not made a statistically significant difference (p>0.05). The differences in the effect of treatment between the two groups were not significant (p=0.12), indicating that using acupuncture with or without catgut embedding might got almostly the same effects in these short 5 weeks. Because the limitation such as strict inclusion criteria, some refused randomization, high drop-out rate and small population size, the therapeutic effect of acupoint catgut embedding for females with simple obesity is not precisely discovered. However, it still indicates that might have a potentially beneficial effect clinically.
