針刺井穴配合低強度雷射治療頸椎病－病例報告

頸椎病屬於一種勞損造成的退化性疾病，保守療法及手術治療對於頸椎病仍有未盡完善之處。本文報告一位50歲男性因第3至6頸椎椎間盤突出而壓迫神經根，導致神經根病變，表現右上肢疼痛、麻木等症狀，並且出現嚴重無力及肌肉萎縮，影響日常生活功能，如刷牙、刮鬍等動作均無法完成。在拒絕西醫手術介入治療的情況下，接受針刺井穴（少商、商陽、關衝、少澤）合併低強度雷射治療（Handylaser Trion, gallium aluminum arsenide, 150 mW, 810 nm, 5 W/cm^2, pulsed wave, 3J）應用於相應節段之頸夾脊穴，每週2次。治療五個多月（46次）後，患肢的疼痛與麻木明顯改善，力量恢復，肌肉萎縮部分亦逐漸回復，病患恢復日常生活功能，更恢復負荷繁重的農務工作。由此可見，針刺井穴配合低強度雷射可應用於頸椎病的輔助或替代治療。

Cervical spondylosis is a type of degenerative disease caused by strain. The conservative and surgical interventions for cervical spondylosis still have some flaws and require improvement. We report the case of a 50-year-old man who had radiculopathy caused by nerve root impact injury due to intervertebral disc herniation at C3-C6. The patient experienced pain and numbness as well as severe weakness and muscular atrophy in the right upper limb, making it difficult for him to perform daily activities such as brushing teeth and shaving. The patient declined Western treatment involving surgery and instead underwent acupuncture at the Well points (LU11, LI1, TE1, and SI1) combined with lowlevel laser therapy (Handylaser Trion, gallium aluminum arsenide, 150 mW, 810 nm, 5 W/cm^2, pulsed wave, 3J) at the neck Jiaji points of the corresponding segments twice per week. The pain and numbness in the affected limb improved after 46 therapeutic sessions in five more months. His strength was restored and the muscular atrophy gradually healed. The patient regained the ability to perform daily activities and returned to strenuous agricultural work then. Thus, the results indicate that acupuncture at the Well points in combination with low-level laser therapy can be used for the complementary or alternative treatment of cervical spondylosis.
