Chemical meningitis induced by a ruptured cyst of craniopharyngioma is rare. This report concerns a 51-year-old male who initially suffered from chronic headache with aggravation, visual disturbance and neck stiffness. A computed tomography of the brain was done at different stages of the disease course. The patient was treated for bacterial meningitis at first, but repeated cerebrospinal fluid cultures were all negative. Finally, after surgical intervention, the pathologist diagnosed as a ruptured cystic craniopharyngioma. The CT images in different stages are presented and discussed.
