Risk Factors for Warthin's Tumor of the Parotid Gland

BACKGROUND: Although Warthin's tumor is believed to be the second most common parotid gland tumor after pleomorphic adenoma, the clinical experience of this research indicates otherwise. This study analyzed the distribution of parotid tumor and examined possible contributing factors of Warthin's tumor. METHODS: This retrospective study analyzed eighty-two consecutive patients who had undergone parotidectomies between April, 2002 and March, 2007. Patient data, including age, gender, history of tobacco use, tumor size, clinical manifestations and histopathological diagnoses, were analyzed. RESULTS: The most common parotid tumor was Warthin's tumor (41.5%) followed by pleomorphic adenoma (23.2%). Mucoepidermoid carcinoma and lymphoma were the most common malignant parotid tumors, accounting for 9.8% and 4.9% of all parotid tumors, respectively. Of five (14.7%) patients with bilateral Warthin's tumors, three (75%) of four patients with either HBsAg or anti-HCV test revealed hepatitis B or C infections. Tobacco use (p<0.001) and hepatitis B and/or C infections (p=0.048) were significantly associated with clinical risk of Warthin's tumor as well as lower incidence of pleomorphic adenoma. CONCLUSION: The most common parotid gland tumor is Warthin's tumor, rather than pleomorphic adenoma. The ratio of Warthin's tumor to pleomorphic adenoma in this series was 1.8:1. In addition to older age and male predominance, smoking and hepatitis B and/or C infections are contributing factors.
