A Carotid Artery Abutting T4bN3M0 Hypopharyngeal Cancer Patient Successfully Treated by ＂Rolling-stone＂ Resection and Chemoradiation-Case Report

摘要

For head and neck surgeons, surgical management of advanced hypopharyngeal cancer with carotid artery involvement is always a big challenge. We report a patient of T4bN3M0 hypopharyngeal cancer, for whom en bloc tumor excision using a ＂rolling stone＂ resection method was performed in 2005. Left side neck dissection followed by laryngopharyngectomy and then right side radical neck dissection looked like rolling a stone (tumor) out of the neck. After adjuvant chemoradiation, the patient achieved a 5-year survival without loco-regional recurrence or distant metastasis, and overall survival of 8 years. For a huge hypopharyngeal cancer, our method is a feasible way to perform surgery.

對於頭頸外科醫生來說，緊鄰頸動脈的T4bN3M0下咽癌的手術治療始終是個巨大挑戰。此篇病例報告為1名患有T4bN3M0的下咽癌病人，腫瘤瀰漫性侵犯兩側頸部及下咽，就像一顆巨石在前頸部，在2005年我們使用“滾石”切除方法進行整體腫瘤切除。手術從左側功能性頸部淋巴廓清開始，連續全喉和部分咽切除及右側根除性頸部淋巴廓清，整個切除的標本好比一顆石頭從左側向右側滾動，與殘留的正常組織成功分離。在接受輔助性放化療後，患者手術5年內無局部復發或遠處轉移，整體存活8年。對於巨大的下咽癌，我們的手術方式是個可行的做法。
