Localized Corneal Amyloidosis-Case Report

摘要

我們報告一先天性德國麻疹徵候群合併先天性青光眼及白內障之病例，雖經三次青光眼手術，眼壓仍無法良好控制，導致角膜混濁及視力喪失，後來接受角膜移植手術取下病變之角膜，病理檢查，意外發現角膜表淺層有澱粉樣物質沉積，並經剛果紅染色及偏光顯微鏡發現特殊之綠色雙極光現象，証實為角膜澱粉樣變性病。
除了柵狀角膜失養症外，這種局部性角膜病變較為罕見。臨床上發現長期眼疾造成角膜混濁者，應懷疑局部性角膜澱粉變性病發生的可能性。

We reported one case of congential rubella syndrome with complicated glaucoma and cataract who had sufferred from long term poor controlled glaucoma and received glaucoma surgery for three times. After penetrating keratoplasty, corneal amyloidosis was found by histopathologic studies (Congo-red stainning and green birefrengence phenomenon). Amyloid deposits was found in intraepithelial, sub-epithelial and superficial stromal layer. From the history, systemic examinations, clinical ocular findings and histopathologic studies, secondary localized corneal amyloidosis was disgnosed.
Excluding lattice dystrophy, the localized corneal amyloidosis is reported rare, nevertheless, long-term ocular disorders with corneal opacity should be suspected this possibility.
