氬氣雷射紅膜穿孔手術後角膜代償不良
Corneal Decompensation after Argon Laser Iridotomy
摘要
氬氣雷射虹膜穿孔術雖然被認為是一種安全且併發症少的治療方法，但其術後造成的角膜代償不良，對於視力的影響卻是值得重視的。本篇報告五例術後角膜代償不良的病例，並探討其造成的可能因素。造成這五位病例角膜水腫的因素大致和Dr. Schwartz所提的青光眼急性發作、點狀角膜(Corneal guttata)、糖尿病和多次接受氬氣雷射等因素相類似。但是二例患眼在移植角膜後的病理檢查，均呈壞死性角膜病變(Necrotizing Keratopathy)現象。

The corneal decompensation is a rare complication of argon laser iridotomy but it is visual threatening. We collected five cases of progressive corneal decompensation after undergoing argon laser iridotomy for angle closure glaucoma. The predisposing factors of these cases included episodes of glaucoma attack, corneal guttata and multiple treatments with high laser energy. Two of these eyes have received corneal transplantation and the histopathological study showed necrotizing keratopathy of the recipient button.
