抗青光眼藥物對結膜下纖維母細胞的影響

The Influence of Antiglaucoma Medications on Subconjunctival Fibroblast

摘要


長期使用局部抗青光眼藥物會降低日後青光眼濾過手術(Glaucoma filtering surgery)的成功率,而青光眼瀘過手術之成功率與術後結膜下纖維母細胞之增殖有密切關係。為進一步了解抗青光眼藥物對纖維母細胞的直接影響,我們測試在不同濃度的Timolol及Pilocarpine作用之下,纖維母細胞生長的情形,另外以Trypan-blue標定死細胞之方法,進一步測定這些藥物對纖維母細胞可能的毒性作用。實驗結果顯示:在任何濃度下,Timolol及Pilocarpine對纖維母細胞都無促進生長的作用;但Timolol濃度高於0.125%、Pilocarpine濃度高於0.1%時,纖維母細胞之增殖明顯地會受到抑制,藥物濃度越高、浸泡時間越長,抑制作用也越明顯;0.5% Timolol及4% Pilocarpine只須9小時,便開始對細胞產生毒性作用。向這些結果我們推論:臨床使用的Timolol及Pilocarpine並不會直接引起結膜下纖維母細胞的增生,長期點藥引起的結膜、角膜或淚液的改變,可能才是造成濾過手術失敗的主要原因。

After long-term use of topical antiglaucoma medications, the successful rate of glaucoma filtering surgery is depressed significantly; this study was designed to investigate the direct effect of antiglau coma medications on subconjunctival fibroblasts. Different concentrations of timolol and pilocarpine were added into the culture medium of human subconjunctival fibrblasts. After 1 or 14 days, cell viability and cell proliferation rate were determined seperately. The results showed that at clinically used concentration both timolol and pilocarpine were cytotoxic for subconjunctival fibroblast, cell death being noted 9 hours after drug addition. Cell proliferation was inhibited also by 0.1% pilocarpine or 0.125% timolol. This inhibitory effect became more apparent when incubaion time was increased. All of these findings indicate that timolol and pilocarpine all have inhibitory effect on subconjunctival fibroblast proliferation. The increased cell proliferation after long-term topical antiglaucoma medications must be caused by some indirect effects of topical medications on conjunctiva.
