Retinal Detachment Associated with Atopic Dermatitis-A Case Report

異位性皮膚炎合併視網膜剝離症-病例報告

摘要


異位性皮膚炎為好發於學齡前孩童的慢性皮膚疾患,報告病例以日本佔大宗。除了皮膚上的表現,此疾病亦會引起眼部的併發症,造成日後的視力障礙。我們報告一位十七歲的女生,自小罹患有厲害的異位性皮膚炎,因左眼飛蚊症急遽增加來求診。眼底可見顯側淺平的視網膜剝離,並於顯側玻璃體基部找到一巨大網膜裂孔(oral dialysis),此外亦合併有兩眼白內障,前房發炎及眼壓偏高的癥狀。經施予左眼鞏膜扣壓術併局部冷凍及雷射治療和兩眼白內障摘除併人工水晶體植入手術,網膜復位狀況良好,目前兩眼視力皆可達1.0以上。

Atopic dermatitis is a heritable, allergic inflammatory reaction characterized by dry and itching skin. Ocular complications of atopic dermatitis include cataract, blepharitis, keratoconjunctivitis, keratoconus, iritis, and retinal detachment. We report a case of atopic dermatitis associated with retinal detachment in a 17 years old female who presented with sudden onset vision blurring and floaters in the left eye. Fundus examination revealed a temporal shallow retinal detachment caused by a large temporal 120-degree oral dialysis. Bilateral anterior chamber reaction and complicated cataract were also noted. The retina was successfully reattached with encircling scleral buckle. The patient subsequently received lens aspiration and posterior chamber lens implantation in both eyes. Her visual acuity was 1.2 in the right eye and 1.5 in the left eye after a one year follow-up. Management of this case and a review of the uncommon association between retinal detachment and atopic dermatitis are discussed.
