摘要

目的：報告一例車禍外傷後造成眼窩內木材異物經過近二個月後始被發現而取出的病例。
方法：病例報告
結果：43歲男性，騎機車發生車禍，跌入一樹叢裡，造成嚴重的顏面撕裂傷及腦挫傷，在他院治療一個多月後，右眼瞼靠近眼窩頂仍有一未癒合且流膿的傷口。後因頭痛劇烈到本院治療。術前經磁振造影疑為眼窩內膿瘍，故安排手術欲行切開引流，術中卻發現原來是一長約6.5公分之樹枝，取出後傷口在一星期內即癒合。
結論：臨床上若發現病人眼眶周圍在外傷後有不能癒合或有分泌物的傷口，應強烈懷疑是眼窩內異物。木材異物在影像檢查(Imaging Study)中常呈現不同風貌，所以容易被誤診，而延誤治療時機。本文提醒大家面對眼瞼周圍有撕裂傷的病患時要時時提高警覺是否有眼窩內異物的可能性。

Intra-orbital wooden foreign body is a rare complication of eye injury, which, if left unrecognized, might lead to potential consequences. We reported a 43 year-old patient who fell off into a bush, suffering from severe facial laceration and brain contusion. After more than a month's treatment, purulent discharge was noted from an un-healing wound on the upper right eyelid. Magnetic resonance imaging revealed a well-defined lesion in the upper right nasal orbit. An abscess was suspected, which turned out to be a 6.5-cm long twig confirmed by explorative operation. The case serves as a reminder that an intra-orbital wooden foreign body should be suspected in any lid trauma with un-healing and discharging wound.
