青光眼網膜神經纖維層的臨床評估Ⅶ原發性隅角開放型青光眼之網膜神經纖維層缺損與視野缺損及眼壓變化之相關性

Clinical Assessment of Nerve Fiber Layer in Glaucoma (Ⅶ) Correlation Changes of Nerve Fiber Layer Defects with Visual Field Defects and Intraocular Pressure in Primary Open Angle Glaucoma

摘要


目的:評估原發性隅角開放型青光眼病患之定性化網膜神經纖維層缺損與定量化視野缺損及眼壓變化之相關性。 方法:隨機選取20名持續追蹤5年之原發性隅角開放型青光眼病患共40隻眼睛,網膜神經纖維層缺損分為細隙型缺損(slit defect),楔狀缺損(wedge-shape defect),及廣泛性萎縮(diffuse atrophy)。視野缺損以Advanced Glaucoma Intervention Study(AGIS)計分系統計分。不正常眼壓頻率以5年內眼壓超過21毫米汞柱次數與所有返診測量眼壓次數之百分比來計算。 結果:共有24隻眼睛網膜神經纖維層缺損變差,16隻眼睛網膜神經纖維層缺損不變。不正常眼壓頻率在上述兩組中,並無顯著差異(T-test p=0.06)。網膜神經纖維層缺損的政變與視野缺損計分變差和不變或變好組之關係中,亦無顯著差異(T-test p=0.41及0.85)。而不正常眼壓頻率在視野缺損計分變化之兩組间,也無顯著差異(T-test p=0.38)。 結論:五年的縱系列追蹤研究,原發性隅角開放型青光眼之網膜神經纖維層缺損變化與視野缺損變化及不正常眼壓頻率無顯著的關聯。

PURPOSE: To evaluate the correlation changes between qualitative retinal nerve fiber layer(NFL) defects, quantitative visual field(VF) losses and intraocular pressure(IOP) in patients with primary open angle glaucoma(POAG). METHODS: Forty eyes of 20 patients with POAG regularly followed up for at least 5 years were reviewed. Qualitative NFL defects were graded as slit defect, wedge-shape defect, diffuse atrophy and combined type. According to AGIS scoring system, all the patients of VF losses were divided as the VF scores worsening group and the VF scores stationary or better group. The abnormal IOP frequency was the ratio of the numbers of IOP above 2lmmHg over the total numbers of IOP in 5 years. RESULTS: There were 24 eyes in NFL defects worsening group as well as 16 eyes in NFL defects stationary group. The abnormal TOP frequency in both NFL defects groups showed no statistically significant difference (T-test p=0.06). There was no significant correlation between the changes of VF scores and NFL defects(T-test p=0.85, 0.41). The abnormal IOP frequency in both groups of VF scores changes also had no statistically significant difference (T-test p=0.38). CONCLUSIONS: The changes of qualitative NFL defects were not significantly correlated with changes of VF scores and abnormal TOP frequency in 5 year followed up period in primary open angle glaucoma.
