台北市北區慢性精神病患心理衛生需求未滿足相關因素之探討

Factors Related to Unmet Mental Health Needs of Chronic Mental Patients in the Community in Northern Taipei City

摘要

本研究的目的爲瞭解社區內居家慢性精神病患心理衛生需求未滿足情形及其相關因素，係以結構式問卷訪問調查的方式，訪談病患或其主要照顧者。研究樣本選自台北市北區三個衛生所列管的精神病個案，共100名。研究結果發現平均每位個案有3.9項次之心理衛生需求未滿足，需求已滿足與未滿足之比例爲1:5.13;居家精神病患心理衛生需求未滿足之相關因素中，經由逐步迴歸分析發現以整體精神狀態較差、年齡較輕、未定時定量服藥及沒有工作之病患有較多的心理衛生需求未滿足的情形。研究樣本中有53位病患之社會功能有中度以上的障礙，有29位口頭表示只願接受居家精神衛生服務，故期望專業人員能到家中提供護理照顧。依本研究結果建議對於居家慢性精神病患之心理衛生需求，應提供多樣性的精神衛生服務，尤其精神科專業人員之個案居家護理處置，定可改善其心理衛生需求滿足的情形，進而提昇居家精神病患的社會功能，降低疾病之復發。

The purposes of this study were trying to reveal the unmet mental health needs and related factors of chronic mental patients in the community. It was a structural questionnaire survey. Data were collected from patients and caregivers by interview. The study samples of 100 chronic mental patients were selected from public health centers in three communities in Taipei city. The outcome showed that, on average, each patient had 3.9 unmet mental health needs, and the ratio of met to unmet health needs was 1:5.13. By using stepwise regression analysis, it was found that the factors related to the unmet needs of mental health services are patients with poor global assessment function, relatively youth, poor drug compliance, and joblessness. 53% of patients had moderate social dysfunction and 29% of families expressed that they would rather keep their patients at home and take care of their patients themselves with some professional support analog to home care service. It is concluded that we can either provide different mental health services to meet different patients’ needs or provide some home care services. Then, we can expect more mental health care services satisfaction, reduced relapse rates and also increased patients’ social function.
