長程巴士旅行引起之腿部深層靜脈血栓

摘要

腿部深層靜脈血栓可能發生在三至四小時的短程旅行中，雖然文獻報告中長程旅行者較易發生此類事件。我們報告一例過去健康情形良好的青年，在巴士旅行後發生嚴重左腿腫痛，診斷為左腿深層靜脈血栓。接受靜脈肝素及隨後口服抗凝血劑(coumadin)治療的六個月，後續追蹤的一年無其他併發症。因為台灣的地理特性，一般旅程所花的時間不長，故就我們所知，在台灣尚未有此類病例報告。因此，臨床醫師仍須警覺到旅行後發生腿部深層靜脈血栓的可能性，並瞭解其預防之道。

Deep vein thrombosis(DVT) can occur on trops as short ad three to four hours even though it has been reported that long distance travelers are more susceptible to this insult. We present a previously healthy 17-year-old man who suffered from marded left leg pain and calf fullness after a long trip on a bus. Under a diagnosis of deep vein thrombosis of the left lower leg, he received intravenous heparin which was changed to oral coumadin for six months. He had had no further episodes after one yeaer of follow-up. To our limited knowledge, thrombotic episodes related to bus trips have not been reported in Taiwan where distances traveled are usually not long because of the size of the islnad. Therefore, physicians should keep this possibility in mind ans be aware of preventive measures.
