以民航客機定期航班經濟艙擔架位方式長時間遠距離轉送依賴呼吸器病患之經驗：個案報告

摘要

加護病房使用呼吸器病患之國際空中醫療轉送，是一項艱鉅的任務挑戰。空中醫療轉送病患可以民航醫療包機、醫療專機或民航客機定期班機經濟艙擔架位等三種方式中的一種來進行。本文報告一位60歲男性，經主動脈全弓置換術後病患，由轉診接受醫院主導安排，以民航客機定期班機經濟艙擔架位方式，經長時間、遠距離，加上不同航空公司兩段航程，轉送成功之案例。因此，建議無醫療急迫性之重症病患，可以民航客機定期班機經濟艙擔架位方式轉送；除非有醫療急迫性之重症病患，才被建議以醫療專機(air ambulance)或醫療包機(medical charter)方式轉送。

Overseas transportation of a ventilator dependent patient is a tough challenge. International medical repatriation can be accomplished by one of three approaches, charter flight, air ambulance or a stretcher in the economy class of a scheduled civil flight. This article describes our experiences when repatriating a 60 years old male who had received Bentall's procedure for aortic dissection. We accomplish this transfer by arranging a stretcher in the economy seating of scheduled civil flights aboard two different Airlines that traveled across three countries. We overcame the need for long duration ventilator support as well as solving the problem of transfer between two airlines and a wait in transit in a third country. Based on our experience, we suggest that patients who are not a medical emergency can be transported by civil aviation. This will reduce the cost of repatriation of noncritical patients and air ambulances or charter flights can then be used for critical patients.
