Pseudallescheria boydii is an opportunistic pathogen of immunocompromised patients. While it is still rare in immunocompetent patients, the number of reports of P. boydii colonization (mycetoma) or infection of the pulmonary cavity by this pathogen have increased due to improved detection methods and increased numbers of immunocompromised hosts. The optimum treatment for this infection remains unclear. Herein, we present a case with disease progression from mycetoma to peri-cavity lung abscess. In this case, P. boydii was isolated from a mixture of pathogens. The infection was successfully treated with voriconazole and adjunct percutaneous drainage and lavage.
