3-dimensional (3-D) Template Model-A Useful Adjuvant Tool in Secondary Ear Reconstruction

3-D立體模型-在耳朵二次重建上的應用

摘要


在小耳症的耳廓重建過程中不佳的結果時常遇到,所以會執行耳廓的二次重建。對於經驗較少的重建外科醫生,使用一個完整的3-D立體模型能更準確地評估皮瓣的情況,特別是在二次重建的病人,主要是在於壞死的表皮、傷疤組織及失去彈性的真皮層。使用3-D立體模型有下列好處,一、能引導創造軟骨框架更複雜的部分;二、把3-D立體模型安置在皮瓣袋內,然後抽吸針置入皮瓣袋,可以預先看出皮瓣是否合適,三、無論是否有經驗的醫生,透過預先的觀察可以減低雕刻和測試的時間,並在雕刻軟骨框架時提供絕佳的參考。基於上述原因,我們提出了3-D立體模型應用於耳朵的二次重建,這篇文章報告了二年內八個病例使用3-D立體模型在耳朵的二次重建上成功的經驗。

Unfavorable results in reconstruction of the auricle for microtia can be encountered from time to time, in which secondary reconstruction of the auricle is usually performed. For the less experienced reconstructive surgeon, use of a complete three-dimensional template would theoretically allow one or more accurately estimates of skin flaps, especially in secondary reconstruction due to the scar tissue, necrotic skin and loss of tensility in subdermal layer. Using the 3-dimensional template as reference may have several benefits as follows: 1. guiding one in the creation of the more complicated portions of the cartilage framework, 2. helping surgeon to assess the adequacy of enveloping skin flap 3. aiding inexperienced operators by reducing carving and assembling time as well as providing a reference in the fabrication process of cartilage framework. For these reasons, three-dimensional templates were created in secondary auricular reconstruction. This paper presents the 2-year experience with 8 cases of secondary auricular reconstruction in which three-dimensional templates are used as an intraoperative aid.
