Exploring the Effectiveness of Discharge Planing Service for Neurosurgery Patients in Medical Center

摘要

神經外科病患因傷害或病況造成身體的殘障，需要長期的照護，而護理人員是病患住院期間接觸病患最頻繁的對象，若能在住院早期即給予持續性的照顧計劃，不但可提昇照護品質亦可降低日益高漲的醫療成本。出院準備服務是衛生署的推動計劃之一，神經外科病房在既定政策下推行此方案，依照結構、過程及結果評值的理念架構下，建立了組織提構、工作流程、計劃及追蹤評值的工具。在推行效益上發現推行出院準備服務後，病患對護理服務很滿意的前三項項目爲：指導您出院藥物服用法、醫生向您解說疾病病情、幫您預約下次門診時間。而不滿意項目有三項爲：協助您租購醫療輔助器材、協助您解決經濟問題、協助您出院後照護環境改置。因出院準備服務工作的推動，提供後續照顧的準備，增加了病患出院的意願，故在病患平均住院天數及超期住院率皆有降低現象。未來之努力方向有：加強民眾及醫護人員對出院準備服務的認知、積極建立後續轉介照護機構、建立相關資源網路資料、成立中立執行小組協助解決轉介困難的個案。

Neurosurgery patients caused their physical deformity on account of injury or sickness, which need to a long-term care and medical stuffs are to make contact with the patients most of the time during in hospital. Supposing that being hospitalize in the early time and getting continuous care plan, not only rising care quality also reduce the increasing cost of medical in days. Discharge plan is one of the programs for the Bureau of Public Health. Neurosurgery wards promote this project follow the policy. According to the structure, process and outcome indicator, constructed framework, working procedure, plan and tracing evaluation's tool. On the efficient promotion: after presenting discharge plan discovered that patients to the satisfaction of nurse service and required items, the average of being hospitalized days and over stage of being in hospital has decreased. The direction of our future afford is to strengthen citizens and medical stuffs for the knowledge of discharge plan, constructing aggressively the following care institution, setting up the relative resource internet data, building up neutral executing group to help dealing with the difficulty of transitional case.
