摘要

本文以洪(2006)FFAM家庭評估模式爲主幹，進行個案家庭的創意護理過程，發現個案家庭有因應失能和溝通的問題，而個案本身有無效性健康維護能力／特定於用藥方面和沐浴自我照顧能力缺失的問題，及孫女的行爲、飲食習慣、和體重等問題，本組成員計畫以家庭中個案需求爲主軸，設計四項創意護理活動，統稱爲「丫清的圓夢」，包括「一目了然防潮筒」、「不用砸人鈴」、「五元圓夢椅」、「喜新厭舊舒服窩」，透過此四項創意護理活動，個案表示完成其洗澡的心願，也改善夫妻間的溝通方式，更增進個案用藥的管理，及身體和睡眠環境的舒適程度，藉由護生們對低社經家庭的護理過程，期許能使個案及其家庭成員間感到人們的關懷和溫暖。

Hsui-Chi Hung (2006) FFAM was used as the core of family assessment model to conduct the family creative nursing process. The family has the problems of the ineffective copying and communication. The case has the problems of the ineffective health maintaining/especially in using medicines and bathing self-care deficit. The granddaughter has the problems of behavior, eating habit. and body weight. The members of the caring team focused on the case of the family and designed four creative nursing activities integrated as ”the dream of A Chin”, including ”understanding and preventing humid of bottle”, ”bell without hitting”, ”five dollars of dream coming true chair”, ”joyful new and hateful old of comfortable bed”. Through the four creative nursing activities, the case completed his wish, changed the way they communicated, improved the management of medicines and comfort levels of physical and sleeping environment. By way of the nursing process for the low social-economic family from nursing students, it is expected the case and family members can feel the caring and warm of human beings.
