實證醫學於實習醫師臨床訓練之應用

摘要


本研究目的在應用實證醫學於實習醫師之臨床訓練,以了解其實用性,並比較 實證醫學資料庫與傳統醫學資料庫對學生引用文獻的能力是否有顯著的影響。以台 大醫院家庭醫學部實習醫師為研究對象,每組給予一小時的實證醫學簡介。實驗組 除了說明實證醫學的原則以外,並指導使用實證醫學資料庫的方法。每個學生在病 房實習期間都要做一次個案討論,由二至三位教師依研究小組設計的問卷評分。本 研究為雙盲實驗,學生及評估教師皆不知道何組為實驗組或對照組。結果顯示在相 同的教學時間及使用相同的網路教材下,實驗組比對照組更能清楚描述文獻的內容, 包括方法及結論。在10個評估項目中,實驗組有6項80%的學生被教師認為同意或 非常同意其對於實證醫學的了解。學生說明查閱文獻所使用的關鍵字、判斷文獻的 種類及評判文獻可信度的能力為得分較低的項目。

This study aimed to evaluate the effectiveness of implementing Evidence-based reviews into the Evidence-based medicine teaching of interns. In the Department of Family Medicine, National Taiwan University Hospital, a one-hour EBM introductory course complemented by web-based teaching materials was delivered to each group of students. The study group underwent EBM reviews, whereas the control group did not. Every student presented their case at case conferences and teachers performed evaluations using the questionnaire designed by the research team. Neither students nor teachers had advance knowledge of the group assignments. The results showed that EBM reviews helped study group to achieve higher scores for describing the contents conclusions. Eighty percent of students obtained EBM performance ratings from teachers of “agree” or“agree strongly” in six of the ten evaluation items. Students received lower scores for three items: describe literature search strategy, identify kinds of literature and determine validity of literature.
