某醫學中心實施床邊教學之成效

Evaluation of Bedside Teaching in a Medical Center

摘要


本研究目的在了解本院參與床邊教學老師及學生(住院醫師及實習醫師)對實施成效之滿意度及相關意見的蒐集分析。本研究方法採用結構式問卷,共發出老師問卷110份,學生135份,回收有效問卷共155份(老師72份,學生83份),回收率分別為65.5%及61.5%。研究結果發現,老師認為擔任床邊教學老師應具備的條件以教學熱忱、專業技能熟練及接受過臨床老師教育訓練所佔比例最高,教學內容以醫病關係、身體檢查及病史詢問比例最高,教學最常遭遇的困難為學生臨床技巧不足、老師工作量大無法擔任教學及學生基礎醫學能力不足。學生認為接受床邊教學後七成以上學生自覺醫療專業技能及醫療問題解決能有進步、教學過程中會主動積極參與及教學後較不怕面對病患。

In order to elevate the quality of clinical education and improve the basic clinical skills of physicians, in March 2002 this hospital started collaboration with the Department of Health to implement the “Ambulatory and inpatient Care Education Subsidy Program”. The objective of this study is to under stand the level of satisfaction of this hospital’s bedside teaching instructing instructing physicians and students (resident and intern physicians) towards the effect of implementation of the program, as well as to collect relevant suggestions. The study made use of structured questionnaires, of which 110 were sent to instructing physicians and 135 to students. The number of returned and effective questionnaires was 153 (physicians 70 and students 83), giving a response rate of 63.6% and 61.5% respectively. The study found that most physicians believed a bedside teaching physician should be enthusiastic, possess good professional skills and have received training from a teacher in clinical skills. With regard to teaching content, doctor-patient relations, physical examination skills and medical history inquiry skills scored highest. Problems encountered mostly during ambulatory care education were students’ insufficient clinical skills, physicians’ inability to teach due to high work load, and students’ insufficient basic medical abilities. Of the students who received bedside teaching, over 70% indicated their professional medical skills and problem-solving skills had improved, that they would actively participate in the teaching process, and that after having engaged in bedside teaching they wee less afraid to confront patients. In order to realize bedside teaching, it is most important establish a teaching objective and consensus and to plan course design, implementation method and clinical skills evaluation method. At the same time, in accordance with government policy, the authorities should engage in continuous examination of the system. (Full text in Chinese)
