呼吸治療專科營養師基礎訓練課程之教學成效

Evaluation of a Specialist Training Program Concerned with Medical Nutritional Therapy in Pulmonary Disease

摘要


本研究的目的在探討營養師呼吸治療在職教育之學習成效及影響其學習成效的因素。課程為期兩天,內容包括呼吸道生理、呼吸道病理、呼吸道疾病之營養照顧及呼吸器監測等四個單元。接受教育者為中華民國營養學會會員168人,教學成效評值以課前及課後之知識問卷評量之。以下為140有效試卷之分析結果:1.凡上完課者,學後評量均有顯著進步,同一份試卷之總平均分數由42.3±18.9分增加至66.5±14.4分,進步幅度達50%以上;2.四單元中以呼吸器監測單元得分率由19%增加至61%,進步幅度最大(約121%);呼吸生理得分率由51%增加至67%,進步幅度最小(約31%);3.任職於醫院的營養師其學後評量的平均值高於非任職於醫院者(69.2±14.3分vs. 62.6±13.8分);4.學習成效不受學員之教育程度、年齡、職場與任職醫院年資等因素影響;5.有28位名次由前1/2群落至後1/2群的學員,其總平均分數雖有進步,由51.6±7.2分增至59.4±10.3分,但進步幅度僅15%。以上結果指出課程訓練對在職教育深具意義,可做為改進課程的參考,以使課程內容務實而符合學員的實際需求。

The purpose of this study was to evaluate a two- day training program curriculum that focused on medical nutritional therapy (MNT) in pulmonary disease. For many years, the Nutrition Society has been in charge of MNT training programs for specialists in various fields. Therefore an evaluation system plays an important role in assessing the quality of the training programs. This curriculum is divided into four sections, which are composed of the physiology of the pulmonary system, the pathology of the pulmonary system, the monitoring of ventilator patients and nutritional therapy for pulmonary disease. Two tests were taken, one before and one after the completion of the curriculum, and the results of these were analyzed. Using the average results, it was found that there was a significant improvement between the two tests from an average score of 42.3±18.9 to 66.5±14.4. If the four sections were analyzed separately, the monitoring of ventilator patients part showed the greatest improvement with a gain in score from 19% to 61% and this was statistically significant. In contrast, the physiology of the pulmonary system showed the lowest improvement from 51% to 67%. When the backgrounds of the test subjects were analyzed in terms of the tests results, those who were employed as a dietitian did significantly better than those who were not so employed (69.2±14.3 vs. 62.6±13.5 p=0.002). However, the number of years employed did no influence the test results. In addition, twenty eight subjects obtained worse results for the second test than the first and this particular group needs to be further studied. It can be concluded from this study that intensive courses are useful to dietitians and these programs are effective because those that are employed show significant improvement in their knowledge.
