以資產為本的社區發展策略-社區醫學訓練與社區資源整合應用新思維

Asset Based Community Development-the Application of Community Medicine Training and the Integration of Community Resources

摘要


隨醫學教育的改革,以病人為中心、以家庭為單位、以社區為取向的基層醫療理念,慢慢成為畢業後一般醫學訓練及國家醫療體系的重點,特別是社區導向的基層醫療(community-oriented primary care, COPC)。但常因基層推動者的執行誤差,過度重視「需要」、「問題」、和「不足」,而忽略了在社區早已存在的「資源」、「技術」及「能力」,導致無法幫助㈳區的永續發展,也無法提供社區醫學訓練有效的學習平台。而以資產為本的社區發展(Asset Based Community Development),具有「以資產為本」(asset based)、「以內部為焦點」(internally focused)、「以關係作推動」(relationship driven)的三個特色,有助於整合既有的社區資源,發展社區資源網絡。因此,對於一般醫學訓練中的社區醫學訓練,提供了學員學習「社區資源的整合應用能力」的另一個思考模式,去面對孤立的醫病關係與基層醫療萎縮的窘境。

As part of the restructuring and innovation that have recently taken place in medical education, the concepts of patient-centered and community-oriented primary care have become the focus of general medicine training and the national health care system, especially community-oriented primary care (COPC). The process of COPC is to identify the health problems of residents and then apply the limited medical resources available to provide a holistic care model of health care services to community residents. This utilizes primary care physicians, community hospitals, community medical groups, community public health, and local health units. However, it is not uncommon that the primary care promoters who assess community health problems only focus on what is absent or problematic in the community, but ignore what is present in the community. Hence, asset based community development (ABCD) is an alternative community development path that is characterized by being an asset-based, internally focused, relationship driven process. This can be helpful because ABCD integrates the existing community resources and contributes to constructing a community resource network. For community medicine training, this is an alternative way of helping students to learn about integration of community resources and allows them to confront poor doctor-patient relationships and the withdrawal of primary care services.
