The Effectiveness of a Psychiatric Training Program in Post-graduate Year One

精神科畢業後一般醫學訓練介入成效研究

摘要


目的:2006年7月起,一個月的精神科訓練開始納入畢業後一般醫學訓練選修課程。本研究主要目的旨在了解非精神科第一年住院醫師參與一個月精神科畢業後一般醫學訓練前後,受訓學員在(1)睡眠障礙(2)憂鬱症(3)譫妄症(4)物質濫用及(5)社區精神醫療等五大精神醫學核心能力改變情形,提供未來精神醫學教育參考。方法:本研究為回溯性研究,收集2006年8月至2009年7月共128位受訓學員學習護照之基本資料與受訓前後自評成績等級,以描述性統計及t檢定分析。結果:本研究發現受訓前,學員在社區精神醫療專題自評成績最低;受訓後學員在五大核心能力均有顯著提升,特別是憂鬱症專題提升最多。結論:精神科一個月的畢業後醫學訓練可有效提升學員基本精神科能力,對於學員較弱的社區精神醫學概念,建議從學校醫學生的課程中給予加強。

Objectives: Previous studies have revealed the efficacy of mental health education for non-psychiatrist physicians. In this study, we evaluated the effectiveness of a post-graduate year one (PGY1) psychiatric training program. Methods: We conducted a PGY1 psychiatric training learning-passport review. Demographic and self-evaluation data were collected. Results: For those who completed the entire program and the self-evaluation, there was significant self-rated improvement in the ability to help patients in each of the following areas of: (1) sleep disorders, (2) depression, (3) delirium, (4) substance use disorders, and (5) community psychiatry. Before intervention, community psychiatry yielded the poorest score in self-ratings. After intervention, the self-ratings improved significantly in all five areas, especially depression. Conclusion: The one month PGY1 psychiatric training course was shown to be effective in improving non-psychiatrist physicians' abilities in the five core fields examined.
