Differential ISW MEAW Technique for the Treatment of Adult Skeletal Class Ⅲ Case with Anterior Open Bite

以非對稱之ISW MEAW技術治療成年骨性三級合併開咬之病例

摘要

一位16歲的成年女性主訴咬合不正及咀嚼功能不好至本院求診。經過臨床檢查發現為骨性三級咬合合併前牙反咬及開晚。由於病人拒絕接受正顎手術的傳統治療法，治療計畫為非拔牙治療（但拔除下顎兩側智齒）。藉由ISW wire（日本國立東京醫科齒科大學研發）改良性MEAW技術配合三級顎間橡皮筋的使用，達成促進前牙開咬及反咬的改正。整個治療過程只花了短短半年的時間，並且在動態治療後，達到穩定的咬合狀態。

An adult female (16 years old) came to our clinic with a chief complaint of poor appearance and not being able to chew food well. Clinical examination found skeletal Class Ⅲ molar relationship with anterior crossbite and open bite. Due to her refusal to receive orthognathic surgery, non-extraction treatment except for the bilateral lower posterior horizontal impacted wisdom teeth was adopted. With ISW1-5 (developed by Tokyo Medical and Dental University), the modified MEAW technique was performed to facilitate the correction of anterior open bite and crossbite, combined with the use of Class Ⅲ elastics. Treatment was completed within six months and a desirable occlusion after the active treatment was achieved.
