應用檢驗條碼科技於病人安全的經驗分享及成效初探

Application of Barcode Laboratory System for Patient Safety

摘要


醫療流程相關疏失不僅造成醫療成本浪費,也危害病人安全,本文為應用條碼科技在病人安全上的實務經驗分享。本院於2006~2008年間將條碼應用在給藥系統,使藥囑開立、藥品調劑、藥物核對到床邊給藥都透過條碼把關,將人為疏失機會減至最低。於2010年再將條碼科技延伸至病人檢驗,由檢驗醫囑開立、承接、採檢、送出及接收都由條碼串連,有鑑於檢驗相關錯誤的發生以採集過程多於檢驗分析過程,故本院將「標示檢體」的執行層面提前至檢驗分析前的階段,經比較上線前及上線後8個月不良檢體發生件數,「檢驗單及檢體不符」類的錯誤由上線前49件(51.58%)減少至上線後4件(5.63%)。

Medical workflow-related neglect causes not only waste of medical costs but also negatively impacts patient safety. This article describes an experience of applying a barcode laboratory system to improve patient identification safety. A barcode medication administration system was applied between the years 2006 to 2008. It integrated order entry, medication preparation, and bedside medication administration into a system to minimize human errors. In 2010, the bar-code laboratory system was extended to the clinical laboratory department. Because more laboratory test-related errors occurred during collection of sample specimens than during specimen analysis, specimen labeling using the barcodes started before sample analysis. The barcode labeling was used during the entire workflow from laboratory order entry to specimen delivery to sample examination. After the implementation of the barcode laboratory system, the number of specimen mismatch decreased from 49 to 4 in an eight-month period.
