A Nursing Experience for a Patient with H1N1 Influenza

摘要

本文描述一位感染HlNl新流感個案因接受隔離治療而身心承受壓力之照護經驗。筆者於2009年08月20日至9月3日護理期間運用Roy's適應模式進行護理評估，確立護理問題有因肺炎致低效性呼吸型態，疾病急性期引起體溫過高，媒體報導與不確定感引發焦慮，以及因感控原則入住隔離病房導致社交隔離。護理過程應用跨團隊共擬治療方針；給予個別護理減緩呼吸喘與發燒不適；提供相關訊息與護理指導增加對疾病認知與因應能力減輕焦慮；以關懷、同理、溝通及信仰給予生理、心理、靈性支持，改善社交隔離問題；並運用轉介資源，持續追蹤個案後續健康問。冀望藉此經驗提供日後照護此類個案之參考。

This article describes the nursing experience for a patient with H1N1 influenza who suffered from physical and psychological distress due to isolation. During the nursing period from August 20 to September 3, 2009, four major nursing problems were identified using Roy's Adaptation Model. These problems included inefficient breathing pattern caused by pneumonia, hyperthermia secondary to the acute stage of disease, anxiety from the media report and uncertainty of disease progression, and social isolation for infection control. The nursing interventions included adopting a treatment plan involving a multidisciplinary team; providing individualized care to relieve shortness of breath and discomfort due to fever; offering relevant information and nursing guidance to increase the patient's awareness and coping capacity for anxiety reduction; and supporting the patient in physiological, psychological, and spiritual aspects through caring, empathy, communication, and a desire of improving the patient's anxiety from isolation. The patient was referred to adequate resources for continual follow-ups after patient discharge. This experience serves as a reference for other nurses who take care of similar patients in the future.
