居家照護病患之主要照顧者綜合性需求調查

摘要

本研究係一橫斷式描述性研究，目的在瞭解接受居家照護服務病患的主要照顧者綜合性的需求。研究對象來自高雄市的8所居家護理機構，以家庭訪視會談方式採結構問卷收集資料，共得120份有效問卷。內容包括居家照護服務的需求、病患及主要照顧者背景資料等，問卷並經信度及效度檢定。研究的主要結果：一、主要照顧者在不同問題方面有不同的需求，有需求的項目，最高達54.2%，其程度有很大的差異。二、在照顧病患方面，需求依序為經濟的補助(照顧費用能抵扣所得稅、補助生活費、醫療費、看護費、或醫療輔具的購置費)；實際的服務(有人幫忙作復健、需要時能快速獲得醫護人員諮詢、有交通工具接送等)；照顧的訊息與技巧(協助移位、飲食與腸胃道問題的處理、病危時處理)；與社會福利之申請(喘息服務、醫療器材補助及租借等)。三、照顧者本身的需求依序為參加社交活動(外出透透氣、外出旅遊)、健康促進與維護、情緒支持(參加照顧者經驗分享國體)、與照顧過程各種工作的兼顧。四、與照顧者需求相關的變項因不同層面其自變項各異。本研究結果可作為提供主要照顧者實際服務及介入研究時的參考。

This study was cross-sectional descriptive research. The study aims to understand the needs of givers of care to home-care clients. The subjects were from eight home care agencies in Kaohsiung-city. The researcher used structured questionnaires to collect information on caregivers, delivered by home visits and interviews. Caregivers completed J 20 questionnaires. The questionnaires addressed the need for home care services, and the backgrounds of clients and caregivers. Reliability and validity were tested and accepted. The results of this study were as follows. 1. Caregivers' unmet needs were diverse, the most needed item was specified by 54.2%. 2. Items of dissatisfaction were in the order, financial aids (income tax reductions, living cost subsidies, medical expenses, fees for aides and medical devices); services (rehabilitation, nursing consultancy when needed, and transportation); information and skills to support caring. Transferring, concerning diet, gastro-enteric care, and critical care; social welfare (respite care, reimbursement and rental for medical supplements/ equipment). 3. The needs of individual caregivers included participation in social activities (going out for recreation or travel); health promotion and health maintenance; emotional support (support groups), and work management. This study can serve as a reference for studies of services and intervention in what associated with family caregivers in the future.
