Nursing Experience with a Congenitally Deaf Patient with AIDS

摘要

本文是照顧一位因嚴重腹瀉入院，先天有聽障的愛滋病個案護理經驗。筆者自92年9月26日至92年10月9日擔任個案的主護護士，藉由直接照護、觀察、筆談等方式收集資料，運用Gordon十一項功能性健康的評估，確立個案有腹瀉、營養低於身體需求、焦慮、不遂從服藥等健康問題。針對其問題提供獨特性的護理，例如筆者自行設計簡易的紀錄單張，教導個案評估排便型態是否日漸改善；並收集多樣而美味的圖片，製作成個案專屬的菜單，以提升其食慾；對於擔心病情曝光，更保證尊重愛滋病患的權益，不提供診斷牌；除此還由網路收集愛滋病友的鼓勵留言，使其面對疾病接受治療。期望經由本文與讀者分享照顧愛滋病且有聽障病患的護理經驗。

This article described nursing experience of an AIDS patient with congenital deafness, hospitalized due to severe diarrhea. The duration of nursing care was between September 26, 2002 and October 9, 2002. The methods of data collection included direct care, observation, and personal interview. Based on Gordon's functional health assessment, the patient's health problems included diarrhea, insufficient nutrition for normal bodily requirements, anxiety, activity without endurance, and lack of medication compliance. For these problems, the author designed a unique nursing plan, including, for example, simple and easy record sheets designed by the author, instructions for the patient to assess daily his defecation habits and improvement, if any. The author also designed a recording sheet for the patient with collection of various culinary pictures to stimulate his appetite. The patient's diagnosis tags were also removed from the door and the bed in order to guarantee the patient's right to privacy and not to ensure the respect and interests of an AIDS patient. Encouraging messages from the websites for AIDS patients were also collected for this patient in order to inspire him for treatment. Through this case report, the author hopes to share this rare nursing experience with an AIDS patient with congenital deafness.
