摘要

本文描述照護一位接受重建手術的口腔癌個案之護理經驗，照護期間為99年4月6日至5月20日，藉由直接照護、會談、傾聽、身體評估、及觀察等技巧收集資料，以Gordon 11項功能性健康型態評估，確認個案健康問題有：焦慮、呼吸道清除功能失效、疼痛、口腔黏膜改變、身體心像改變等健康問題。筆者在護理過程中，運用同理心與個案建立彼此的信任關係，傾聽內心感受，針對個案提供個別性及整體性相關照護，改善個案生理的不適，如疼痛舒緩、口腔護理，促進舒適與睡眠，並提供完整的手術前後相關資訊，幫助個案面對癌症的壓力與渡過身體心像改變的衝擊，使個案能學習自我照護，勇敢走出陰霾，同時鼓勵家屬共同參與護理計畫，能繼續往後的治療。

This paper examines an oral cancer patient under the nursing process between April 6, 2010 and May 20, 2010 after a reconstructive surgery. Data were collected through direct care, counseling, listening, physical assessment, and observation. The patient was assessed using Gordon's 11 Functional Health Patterns and was confirmed to suffer from anxiety, ineffective airway clearance, pain, oral mucosal change, and body image change. During the process, I provided individual and overall nursing care and established mutual trust through the use of empathy. I listened and eased the patient's physical indisposition, such as pain release, oral care, and improvement of sleep. I also provided well-rounded information on preoperative and postoperative care and helped the patient deal with the pressure of cancer and the shock brought by the change of body image. The patient learnt about self-care and walked out of shadow. The patient's family was also involved in the nursing care plan and helped the patient face the forthcoming therapy.
