Effects of Pilates Exercise on Chronic Low Back Pain: Systematic Review and Meta-Analysis

彼拉提斯運動對於慢性下背痛之效果:系統性回顧及統合分析

背景與目的:過去鮮少針對彼拉提斯療效之研究;因此,本研究目的為利用系統性回顧及統合分析探討彼拉提斯運動對下背痛的療效,以及提出適當的彼拉提斯運動內容及其副作用。方法:搜尋MEDLINE、PubMed、CINAHL、Web of Science、PEDro、Cochrane library、Chinese Electronic Periodical Service (CEPS),限制為1995到2010年間的隨機對照或臨床對照文獻,使用皮卓分數來分析文獻的品質。符合條件選入的文章由四位有研究經驗的物理治療師兼彼拉提斯指導員為檢閱者,評量文章的品質並取得一致的分數。若有兩篇以上,提供相同變數,就會使用Review Manager Version 5.0 軟體將其列入統合分析。結果:總共有四篇符合條件之研究,皮卓分數結果為3分至8分。統合分析結果顯示彼拉提斯運動顯著減少疼痛的程度(標準均差〔95%信賴區間〕:-0.52〔-0.87,0.16〕,p=0.004)。功能失常程度的改善則無達到顯著(標準均差〔95%信賴區間〕:-0.10〔-0.47,0.28〕,p=0.625)。結論:彼拉提斯運動治療後,疼痛降低較一般治療顯著,功能則無顯著差異。未來應要有更多高品質的研究專注於彼拉提斯對於慢性下背痛的療效、機制做探討。

Background and Purpose: Pilates, a kind of complementary and alternative medicine (CAM) approaches, was frequently applied to treat the symptoms and signs in people with chronic low back pain (LBP). There was little scientific evidence focusing on its effects. Hence, the purpose of this study was to examine the effects of Pilates training versus minimal intervention in people with chronic LBP through systematic review and meta-analysis and to figure out the appropriate content of Pilates exercise and its adverse effects. Methods: We searched MEDLINE, PubMed, CINAHL, Web of Science, PEDro, Cochrane library, Chinese Electronic Periodical Service (CEPS) library from 1995 to 2010. With English and Chinese language restriction, the study quality was evaluated using PEDro scale. Studies matching predetermined inclusion criteria were evaluated the quality by four reviewers and the consensus was obtained. When an outcome was investigated in more than one randomized controlled trial (RCT) or a clinical controlled trial (CCT), a meta-analysis would be applied. The meta-analysis was facilitated by Review Manager Version 5.0. Results: Four studies were identified. There were two RCTs, and two CCTs. The PEDro score ranged from 3/10 to 8/10. The results of the meta-analysis showed Pilates intervention significantly eliminated the pain (standard mean difference [95% confidence interval]: -0.52 [-0.87, -0.16], p=0.004). No significant change of the functional disability was observed (standard mean difference [95% confidence interval]: -0.10 [-0.47, 0.28, p=0.62). Conclusion: Pilates exercise is effective on immediate pain relief but not on function for chronic LBP. More studies with higher quality should be carried out to determine the effects and the mechanism of Pilates exercise in people with chronic LBP.
