摘要


為了讓早產兒有理想的發展預後,以家庭為中心的早期介入方式已逐漸成為趨勢,該介入方式著重於兒童本身外,也強調影響發展的環境因素。而母親通常是嬰兒的主要照顧者,母子互動品質乃被認為是早期的環境影響因素之一。為了能夠讓臨床人員對於早產兒的母子互動有較完整性的瞭解,以幫助介入的設計及療效,本文章的目的為回顧過去早產兒母子互動的文獻,並探討早產兒母子互動的特性與其對於早產兒後續的發展影響、以及提供母子互動衛教的早期介入對於母子互動和早產兒發展的介入效果。綜合文獻資料顯示,早產兒的母親比足月兒的母親較常出現負面的互動行為,例如過度的控制行為、低敏感度和低反應。而這些負面的互動行為與早產兒的不利發展(如認知、語言和行為表現)具有相關。至於提供母子互動衛教的早期介入研究則顯示,對於早期母親互動行為具有療效的介入方式似乎較能帶來後續早產兒的發展預後提昇。總結而盲,以上的資料提供重要訊息以幫助了解早產兒之母子互動,未來的研究需進步探討何種母子互動模式的介入能為早產兒帶來更大的發展效益。

Early intervention based on family-centered model has gradually become a trend for the enhancement of developmental outcome in preterm infants. Such intervention services emphasize on intervening both child and environment factors. Mothers usually serve as the primary caregiver and mother-infant interaction has been considered as an important environmental factor in the early period. Understanding of the quality of mother-infant interact ion in preterm infants would help clinical professionals design effective intervention. The purpose of this article was aimed to review previous studies on mother- infant interaction after preterm birth. We addressed the features of mother-infant interaction in preterm infants, the impact of maternal interactive behavior on preterm infants' development, and the effectiveness of education on mother-infant interaction and child developmental outcome. The results showed that mothers of preterm infants tended to exhibit negative interactive behaviors including more controlling, less sensitivity and less response than mothers of term infants. Furthermore, the negative interactive behaviors were found to associate with later adverse development (e.g. cognitive, language and behavior) in preterm infants. Few prior studies appeared to suggest that earlier intervention effect on maternal interactive behaviors may relate to subsequent better child development. In summary, the findings provide important information to help understand the mother- infant interaction associated with preterm birth. Future study needs to investigate what style of mother-infant interaction intervention best benefits developmental outcome in preterm infants.
