三級異常咬合中臉部複合體之幾何形態測量學分析

Geometric Morphometric Analysis of Midfacial Complex in Class Ⅲ Malocclusion

摘要


幾何形態測量學有別於傳統測顱術分析,可以提供更完整的形態差異資訊。相關研究指出三級異常咬合與中臉部複合體間影響密切。本研究採用中國正常咬合與三級異常咬合男女孩童共100位,來探討兩者中臉部複合體的形態差異部位,其中男、女族群之不同咬合者各25位。透過尺寸與形狀變化分析以及主應變方向分佈的探討後,發現三級異常咬合男、女童後中臉部區域,尺寸比正常咬合小,且都在前後方向上縮短,因此推測此主要的差異可能會造成中臉部具有往後塌陷的特徵。至於在三級異常咬合前中臉部區域,男童尺寸明顯變大,女童大部份尺寸則是變小,而齒槽骨區域可發現男女皆有向前移位的趨勢。此初步結果可提供相關研究之參考,以更瞭解異常咬合顱顏形態之相互影響關係。

The geometric morphometric analysis, comparing with traditional cephalometric analysis, could provide more information on morphometric variations. Many studies had shown that the Class Ⅲ malocclusion is linked with the midfacial complex. The purpose of this study is to investigate the morphological difference between the normal occlusion and the Class Ⅲ malocclusion among Chinese children. One hundred children had been selected for this study and each occlusion has 25 subjects of male and female groups. The results indicated that the midfacial size of the Class Ⅲ malocclusion was smaller than normal occlusion for both male and female groups and a horizontal reduction was observed at the posterior midfacial region through analysis of size and shape changes and distribution of principal strain. Therefore, it could presume that the Class Ⅲ malocclusion in midfacial deficiency was caused by anteroposterior retardation of upper jaw growth. The anterior midfacial of the Class Ⅲ malocclusion was detected in size expansion for the male and reduction for the female. The maxillary incisor alveolus has an evidenced forward deformation for both male and female group. The primary results of this analysis will make guidelines for allied researches so that better understanding of craniofacial development could be obtained.
