病例報告：發生於眼部、齒齦及下唇之犬多發性無菌性肉芽腫

Case report: Sterile Granuloma with Ocular, Gingival, and Lower Lip Involvement in a Dog

摘要

本病例為一隻兩歲大未絕育之雄性混種犬，臨床發現其眼球突出，雙眼結膜紅腫，角膜周邊混濁，並且在角膜中央有類似脂質物質的沉積，角膜與鞏膜交界處有大量黑色素沉著及血管新生。口腔檢查發現其右側上方門齒及犬齒處的頰側黏膜有多發性紅腫，齒齦表面粗糙、紅腫，但無任何潰瘍、水泡或出血病灶；另外在下唇部左前側，有漸進性、全面性的泛紅及腫大。組織生檢顯示所有病灶均為肉芽腫性之病變，經特殊染色及血液檢驗均無特異病原存在，免疫檢驗亦為陰性反應。病患於類固醇投予後，症狀明顯獲得改善。本病例經臨床症狀、病理檢驗及治療反應，其最終診斷為罕見之多發性無菌性肉芽腫。

A 2-year-old, intact, male, mongrel dog was presented with bilateral exophthalmus and hyperemic conjunctiva. Central lipid precipitation, peripheral opacity, neovascularization, and pigmentation were noted on the cornea. Oral examination revealed regional swelling and hyperemia on the right upper buccal and gingival mucosa. No ulceration, cyst or hemorrhage was found. Progressive erythema and swelling of the left lower lip were also noted. Biopsies of the involved tissues revealed uniformed histopathologic features of granuloma. No etiological agents were identified under special staining and hematology. Immunology examinations and test revealed negative result. All lesions were readily responsive to corticosteroid treatment. Based on the clinical presentation, histopathological appearance, and the response to treatment, a diagnosis of sterile granuloma was made.
