一位未婚女性罹患乳癌併乳房自體重建手術之照護經驗

摘要

本文探討一位25歲未婚女性罹患乳癌併乳房自體重建手術之照護經驗。護理期間為2015年6月30日至7月6日，藉由觀察、會談紀錄及病歷收集個案資料，運用生理、心理、社會、靈性等進行整體性評估，發現個案因確診乳癌及接受手術治療，對未來有不確定感，確立有焦慮、組織完整性受損及身體心像紊亂等健康問題。經筆者術前提供圖示法說明，並有可移動式衛教看板告知照護進展，藉由醫療團隊合作，給予支持團體及勵志書籍資訊，且運用個案手機錄影模式提供換藥過程影片，及QR Code連結衛教單張，提升自我照護技能，鼓勵家屬及男友參與治療過程，由他們給予支持，協助個案儘早重返家庭與社會生活，希望提供此經驗做為日後照護類似個案之參考。

This article explores the nursing experience of caring for a 25-year-old unmarried woman undergoing breast cancer with breast autologous reconstruction surgery. The period of nursing care was from June 30th to July 6th, 2015. Through observation, interview records and personal medical data collection, we found the case possessed uncertainty and fear for the future after definitive diagnosis of breast cancer and the following operation, by using physical, psychological, social, spiritual and other holistic assessments. As a result, we confirmed that the patient suffered from some psychological and health problems such as anxiety, damage to the integrity of the organization?, and physical and mental disorders. In order to facilitate the patient’s return to her family and social life as soon as possible, our medical team provided some helpful nursing care skills to the case. For example, we explained the care progress in detail pre-operatively using a graphic method and a mobile health education board. In addition, support group access and inspirational books were provided. As for the post-operative care, we assisted the case in promoting self-care skills with her own mobile phone video mode recording the dressing process and QR app code-linking to health education leaflets. Family members and the boyfriend were encouraged to participate in the treatment process and give support. Hopefully, this nursing experience can be seen as a useful reference for similar cases in the future.
