摘要

本文敘述一位肺癌末期個案於加護病房之護理經驗，護理期間為2015年11月21日至12月1日，筆者於照護期間以觀察、傾聽及病歷資料收集主客觀資料，評估個案生理、心理、靈性及社會等層面，發現個案有低效性呼吸型態、焦慮及預期性哀傷等護理問題。透過主動關懷及持續的支持，與個案及家屬建立良好護病關係，教導放鬆技巧及轉移注意力等非藥物措施減輕個案焦慮情緒，提升個案氧合濃度與舒適度，鼓勵個案表達內心情感，協助家屬達成個案心願，讓個案完成道歉、道謝、道愛與道別之四道，並能為出加護病房後的死亡準備。建議加護護理人員除了加強重症護理之外，安寧療護亦不可或缺，以期運用於癌末個案及家屬，提供最佳之身心靈之照護。

This paper describes the nursing experience of caring for a terminal lung cancer patient in the intensive care unit from November 21 to December 1, 2015. The author collected subjective and objective data from observations, listening and medical record data during the period of care, and assessed the physiological, psychological, spiritual and social aspects of the case. The author found that there were some nursing problems, such as ineffective breathing pattern, anxiety and anticipation of grief. In addition to strengthening intensive care, hospice care is also essential for terminal stage cancer patients and their family in order to provide the best care for both body and mind.
