提升護理人員執行手足護理完整率

Enhance the Complete Rate of the Nursing Staff Providing Hand and Foot Care

摘要


化學治療是癌症治療中具重要療效的一種方式,副作用的預防與處理也同步受到重視,近年來,化學藥物產生的手足症候群問題對病患生活品質造成負向影響,而一個正確完整的手足護理,可以減少因化學藥物引起的手足副作用問題。基於現況發現:(1)護理人員對手足症候群照護認知僅達42%,(2)護理人員執行化學治療病患手足護理完整性僅有29.2%,(3)病患對於護理人員提供的手足照護滿意度僅56%,顯示此項症狀照護品質有待提升,期望藉由專案改善能提升護理人員化學治療手足護理完整率達91%以上。改善方案包括:(1)制定化學治療手足症候群照護標準作業準則,(2)製作手足護理衛教單張,(3)安排人員手足護理在職教育訓練,(4)增設手足護理試用包。本專案藉由設立化學治療手足症候群照護標準及提升護理人員對手足症候群的照護認知,落實護理人員提供病患在執行具手足症候群副作用化學藥物治療期間,一個完整的手足護理,專案執行後,護理人員對化學治療手足症候群的照護認知提升到99%,手足護理執行完整率提升到91.2%,病患滿意度達94.8%。透過專案改善,達提升護理人員執行手足護理完整目的。

Chemotherapy is one of the most effective therapies for cancer treatment. Therefore, the prevention and management of the side effects caused by chemotherapy have to concurrently be attened. In recent years, it has been found that the hand-foot syndrome caused by chemotherapy has led to negative effects on the patients' quality of life. Accurate and complete hand and foot care can reduce the side effects of hand-foot syndrome caused by chemotherapy. The observation for the current practice showed that (1) only 42% of the nursing staff were aware of hand-foot syndrome; (2) only 29.2% of the nursing staff implemented the provision of the complete care for chemotherapy patients' hand and foot; (3) only 56% of the patients were satisfied with the hand and foot care provided by the nursing staff. The data suggestsed that, there was a need to improve the care quality of the hand-foot syndrome. We expect that, through implementing the project, the rate of providing complete food and hand care for the chemotherapy patients could increase to more than 91%. The improvement project included: (1) the establishment of the standard care procedures for the chemotherapy patients with hand-foot syndrome; (2) the development of the health education leaflets on the hand and foot care; (3) the arrangement of the on-job educational training for nurses on the hand and foot care; and (4) the addition of developing the hand and foot nursing care kit. By establishing the standard care for hand-foot syndrome caused by chemotherapy and improving nursing staff's awareness of hand-foot care, this project enabled nursing staff to provide patients with complete hand and foot care during chemotherapy. After the project was implemented, the nurses' recognition rate of the hand-foot care for chemotherapy patients complicated with the hand-foot syndrome increased to 99%, the rate of providing complete hand-foot care increased to 91.2%; and 94.8% of the patients were satisfied with the hand-foot care provided by nurses. The implementation of the project helps the nursing staff to achieve the goal of offering complete hand and foot care.
