比較放射治療骨盆部患者採仰臥及俯臥姿勢的準確性

Comparison of the Supine vs. Prone set-up Accuracy for Pelvic Carcinoma Patients in Radiation Therapy

摘要


本研究針對使用諾力刀(Novalis)治療機治療的骨盆腔患者,比較採仰臥及俯臥姿勢的準確性。用其搭配影像導引定位系統(ExacTrac X-ray positioning system)將X-ray和治療計畫中的數位重組影像(Digital Reconstructed Radiography, DRR)進行比對。擺位誤差可主要分為系統偏差(systematic deviation)和隨機偏差(random deviation)。本研究收集共34名利用即時影像系統來測定擺位偏差的患者資料進行分析。在仰臥姿勢:三維各軸的平移偏差在左右、頭腳及前後方向分別為2.17mm、4.89mm、4.29mm。Σ值分別為1.02mm、1.67mm、2.79mm。σ值分別為0.46mm、1.05mm、1.55mm。在俯臥姿勢:三維各軸的平移偏差在左右、頭腳及前後方向分別為2.61mm、6.97mm、4.92mm。Σ值分別為1.02mm、3.44mm、2.89mm。σ值分別為0.60mm、2.36mm、1.92mm。分析770組研究數據,分析在左右、頭腳及前後方向位移大於1公分所對應發生頻率,在仰臥姿勢分別為0%、11.1%、15.8%。俯臥姿勢分別為0.5%、22.4%、20.7%。最後結果本科患者以仰臥姿勢治療的準確性優於俯臥姿勢。

The study was designed to investigate the supine and prone set-up accuracy for pelvic carcinoma patients treated with Novalis, coupled with ExacTrac X-ray positioning system, to compare X-ray images with Digital Reconstructed Radiography (DRR) in the treatment planning system. Setup deviation could majorly be divided into systematic deviation and random deviation. We collected thirty-four patients and used the ExacTrac X-ray positioning system to analyze the setup data. In the spine position: The mean translation of each dimension in X(L-R), Y(S-I), Z(A-P) were 2.17 mm, 4.89 mm, and 4.29 mm, respectively. The standard deviations of systematic (Σ) were 1.02 mm, 1.67 mm, and 2.79 mm, and that of the standard deviations of random (σ) were 0.46 mm, 1.05 mm, 1.55 mm, respectively. In the prone position: The mean translation of each dimension in X(L-R), Y(S-I), Z(A-P) were 2.61 mm, 6.97 mm, and 4. 92 mm, respectively. The Σ were 1.02 mm, 3.44 mm, and 2.89 mm, and that of the σ were 0.60 mm, 2.36 mm, 1.92 mm, respectively. For all 770 sets of data in this study, In the supine position :the frequency of the shift larger than 1cm in X(L-R), Y(S-I), Z(A-P) were 0%、11.1%、15.8%, and that of the prone position were 0.5%、22.4%、20.7%, respectively. The final results in the supine position accuracy better than prone position in treating our patients.
