Transrectal ultrasonography (TRUS) is the main tool for evaluation of prostate in men with lower urinary tract symptoms (LUTS). The role of TRUS in the diagnosis of bladder neoplasm is controversial. We describe a male patient presented with urinary difficulty and hematuria for several years. He accepted medical treatment but the response was poor. We found an incidental neoplasm in the bladder neck by TRUS during prostate workup. It worked as a check-valve that caused bladder outlet obstruction. The symptoms improved after tumor removal immediately. The pathology showed inverted papilloma of the bladder neck. We suggested that TRUS may play some role in the diagnosis of bladder neoplasm in posterior bladder wall and bladder neck.
